Latar Belakang : Sistem pembiayaan kesehatan yang diterapkan oleh BPJS Kesehatan yaitu tarif INA-CBGs terhadap biaya rill rumah sakit. Seringkali terjadi ketidak sesuaian antara tarif klaim INA-CBGs yang lebih rendah dibandingkan dengan tarif ril rumah sakit, sehingga rumah sakit merasa rugi. Tujuan penelitian : mengetahui gambaran biaya pelayanan kesehatan peserta jaminan kesehatan pada penderita penyakit katastropik dan perbandingan biaya tarif INA-CBGs dengan biaya rill rumah sakit. Metode : penelitian ini menggunakan studi penelitian literatur review, artikel dikumpulkan dengan menggunakan database google scholar dan telah terindeks Sinta. Hasil : komponen biaya yang banyak menyerap biaya adalah biaya akomodasi, biaya tindakan medis, biaya pelayanan medis, biaya penunjang, obat-obatan dan bahan medis habis pakai serta tidak semua klaim yang menggunakan tarif INA-CBGs terdapat selisish tarif dengan tarif Rill RS yang dapat merugikan rumah sakit. Kesimpulan : komponen biaya sangat berpengaruh terhadap tarif pelayanan, tidak semua tarif INA-CBGs dapat merugikan rumah sakit tergantung jenis penyakit yang dialami, apabila di salah satu jenis penyakit terdapat selisish negatif maka ada salah satu penyakit lain yang memiliki selisih positif sehingga rumah sakit tidak rugi jika menggunakan tarif INA-CBGs. Saran : untuk penentuan tarif INA-CBGs sebaiknya dilakukan evaluasi agar tarif dapat sesuai dengan tarif RS dan rumah sakit harus memberikan pelayanan secara efisien agar tidak terjadi pembengkakan biaya.

Keconclusion: the cost component is very influential on service rates, not all INA-CBGs rates can harm the hospital depending on the type of disease experienced, if in one type of disease there is a negative difference then there is one other disease that has a positive difference so that the hospital does not lose if using INA-CBG rates. Suggestion: to determine the tariff for INA-CBGs, evaluation should be carried out so that the rates can be in accordance with the rates for hospitals and hospitals must provide services efficiently so that costs do not increase
