Pembimbing: Diniyah Kholidah, SST, S.Gz, MPH Formulir informed consent adalah bukti tertulis dari sebuah persetujuan tindakan kedokteran sehingga diperlukan untuk memastikan bahwa pasien telah mengerti semua informasi yang dibutuhkan untuk membuat keputusan dan menjadi bukti hukum jika diperlukan. Dalam pengisian formulir informed consent di RSU Universitas Muhammadiyah Malang masih ditemukannya ketidaklengkapan pada identitas pemberi informasi dan atau pelaksana tindakan, waktu dilakukannya tindakan dan catatan penting. Tujuan dari penelitian ini adalah untuk melakukan review identifikasi identitas, review laporan/catatan penting, review autentifikasi dan review pencatatan yang baik dan benar. Metode yang digunakan adalah metode deskriptif dan pengumpulan data menggunakan metode check list. Berdasarkan hasil penelitian dan pembahasan persentase kelengkapan dari masing-masing komponen review, kelengkapan tertinggi ada pada review identifikasi sebesar 90%, sedangkan pada review autentifikasi rata-rata sebesar 88,4%, review laporan/catatan penting rata-rata sebesar 78,4% dan review pencatatan yang baik dan benar rata-rata sebesar 87,8%. Dari penelitian ini dapat disimpulkan bahwa perlu dilakukannya monitoring mengenai kelengkapan pengisian formulir informed consent dan pembenahan pada formulir informed consent agar sesuai dengan ketentuan dan terciptanya kualitas rekam medis yang baik. Mentor: Diniyah Kholidah, SST, S.Gz, MPH An informed consent form is written evidence of a medical surgical agreement to ensure that the patient understand all of its information to make a decision and becomes legal evidence if needed. In filling out the informed consent form at the University of Muhammadiyah Malang Hospital still found incompleteness in the identity of the giver of information and or executor of the action, the time of the action and important notes. The purpose of this study is to carry out a review of identity identification, review of important reports/notes, review of authentication and review of good and correct records. The method used is descriptive method and data collection using the check list method. Based on the results of the study of the percentage of completeness of each component of the review, the highest completeness is in the 90% identification review, while in the authentification review is an average of 88,4%, the review of important reports/notes is an average of 78.4% and the review of good and correct records is an average of 87.8%. From this study it can be concluded that it is necessary to carry out monitoring regarding the completeness of filling the informed consent form and improvement on the informed consent form to comply with the provisions and the creation of good quality medical records.
